How does Josie actually die? I was just reading the Packard Mill essay and it says she died from fear?? Surely, that's too simple for a show like TP.

 Oh, also, I just realised that the name of the insurance salesman that tips off Catherine as to Ben Horne's double-crossing, Walter Neff, is the same name as the insurance slaesman in Double Indemnity, a Billy Wilder film. Also there's a character called Gordon Cole, in Sunset Boulevard, another Wilder film and one of Lynch's favourite films.

Josie's cause of death cannot be determined, according to a discussion between Hawk and Cooper in ep 24:

"This is worse than the bureau, all the international documentation. Eckhardt... Josie... (reaches for a folder) Have you seen Doc Hayward's autopsy on her? Says he couldn't determine cause of death. And the body weighed sixty-five pounds." 

Later (ep 28), Cooper tells Truman about his vision of BOB and LMFAP: 

"She was trembling with fear. I would go as far as to say quaking like an animal. I might venture a guess to say it was the fear that killed her. At the moment of her death, I saw BOB. As if he had slipped in through some crevice in time. Upon reflection, I believe there's a connection between his appearance and Josie's fear, as if he was attracted to it. Feeding off it, somehow."

So it does seem to be "fear" that the series relies on to explain the death.  But it is by no means "simple," as you say.  In any case, I think the guy who wrote the Mill Conspiracies essay is a pretty trustworthy guy... so you can rely on him.  ;)

BOB was able to trap Josie in the wood because she was trembling with fear and it gave him an opening. He did it for the same reason he killed Windom Earl - She, like Windom, tried to kill Coop and that would ruin BOB's plan to possess him. BOB made sure she wouldn't get a chance to shoot Cooper again.

I hadn't thought of it that way before.  The Cooper preservation, that is.  Interesting.  It does seem a little cerebral for BOB, though... but what do I know?  :)

 I've only watched TP once and have never rewatched an episode. I always thought I understood most of what goes on with the Mill but it really is fucking complicated. I never really thought that the Mill was that integral to the show to have it as the intro but it does have the atmosphere that TP has.
